IRS 1095-C Forms

To comply with the provisions of the Affordable Care Act (colloquially known as ObamaCare), Mines
must report health insurance information for 2015 to the I.R.S. and to employees who meet the IRS
definition of a full-time employee. Per the I.R.S., a full-time employee is generally an individual who
worked at least 130 hours for one or more months during 2015. For hourly paid employees, hours are
averaged over the course of the year, and we must report on employees who averaged 130 hours per
month or more. Students work paid through work study funds is exempt from the reporting
If you fall into the I.R.S. full-time employee category, we will provide you an I.R.S. Form 1095-C. We are
currently working on these forms. However, we are waiting on a software upgrade before we can print
and mail forms. Once the upgrade is instal ed, the forms will also be available in Self-Service Banner.
This year’s IRS deadline for issuing forms is March 31, 2016.
Employees with health coverage should also expect to receive a Form 1094-B from their insurer
indicating the months that you and any dependents were insured.
For the 2015 tax year, the IRS is not requiring that you have the 1095-C form in order to file your 2015
tax return. Once received, the form should be kept with your tax documents. For more IRS information
